About Mesotherapy

Weight loss and fat reduction are common concerns among men and women seeking a fit, attractive and healthy appearance. Weight loss can be achieved through lifestyle change including diet and exercise, and in some extreme cases, bariatric surgery. However, many men and women have pockets of fat in areas of their body that do not disappear regardless of the amount of exercise or change in diet.

Surgical techniques including liposuction can target these localised pockets of fat. But surgery is not for everyone. It requires acceptance of significant risk and a potential for complications. It also requires time off work and other activities of daily living.

While many non-surgical techniques are marketed for the treatment of these pockets of fat, very few are actually effective. In the early 1950s in France, physician Dr. Michel Pistor, developed a treatment called Mesotherapy to target cellulite, among other injuries and ailments via a series of small injection.

What is Mesotherapy?

Mesotherapy is treatment wherein a solution of vitamins, minerals, amino acids and a medication called phosphatidylcholine are injected in small amounts into the skin. The goal with respect to fat reduction is cellulite reduction. The results of the treatment are minimal and the number of treatments required to see any change are significant.

In recent years, scientists have built on the principles of Mesotherapy to develop a treatment called Lipodissolve.

What is Lipodissolve?

Lipodissolve evolved from Mesotherapy into a more effective, safer and more advanced therapeutic technique. While Lipodissolve evolved from Mesotherapy, they are actually very distinct treatments. In Lipodissolve, the key active ingredient, phosphatidylcholine is present in the injections in a much greater concentration. In addition, injections are administered directly into the fatty tissue instead of into the skin like in Mesotherapy. During the Lipodissolve treatment, the active ingredient phosphatidylcholine, causes cell walls to burst and then dissolve. Fat from within the cell is released and destroyed by the body.

Does Mesotherapy really work?

Mesotherapy can work for pockets of fat but may take a minimum of 10 sessions for results to be where a patient wants. It is time consuming and costly.

Does Lipodissolve really work?

Lipodissolve works to reduce pockets of fat. Approximately 2.5 cm of fat are eliminated with every treatment. Studies conducted across multiple countries have shown patient satisfaction rates of over 90%.

Are Mesotherapy and Lipodissolve treatments safe?

Both Mesotherapy and Lipodissolve treatment providers claim that these treatments are safe and affordable. Thousands of patients have undergone both treatments, and the products used in both cases are naturally occurring in the body. Lipodissolve is more clinically effective because injections are targeted directly into the fatty tissue.

Should I choose Mesotherapy or Lipodissolve?

While Mesotherapy may appear less expensive at the outset, the number of treatments required makes it less appealing and equally costly. Lipodissolve is quicker and has shown greater efficacy in its ability to provide clinically significant results (ones that you can really see!).

The procedure, also known as lasabrasion, laser peel or laser vaporization, basically involves removing layers of skin using concentrated light energy. Since the introduction of laser skin resurfacing in the 1980s, the technology and methods used in practice have been continuously improved to reduce risks to patients and decrease the time necessary for recovery. Nonetheless, any skin treatment involving lasers can still be dangerous in the hands of inexperienced physicians, or those professionals who are easily swayed by patients who want aggressive treatments for faster results. Even newer technologies like fractional laser skin resurfacing still pose risks to patients who have unrealistic expectations about results or who do not receive honest information from their physician.

Ablative Laser Resurfacing

Ablative laser skin resurfacing uses powerful lasers to destroy the upper epidermis, or the outer layer or skin. This was one of the original uses for photo-energy technology, as the earlier lasers were very much focused on being an alternative to chemical peels and Dermabrasion. One of the first machines for skin laser resurfacing was the continuous wave Co2 lasers, which were very powerful and magnified infrared light waves to deliver destructive beams of energy to surface skin cells. However, although Co2 laser skin resurfacing procedures have been known to produce noticeable results, the earlier devices have also been notorious for their frequent side effects. For example, when the industry was just developing, it was not uncommon for patients to experience, painful skin inflammation, skin discoloration, scarring, and in some cases even infection. One of the biggest causes of problems with early generation ablative laser resurfacing was that it destroyed cells in the epidermis indiscriminately. The entire upper layer in the treatment area would be removed, and the body would scramble to heal skin damage which was in some cases similar to first degree burns. Over the years, laser skin resurfacing techniques would be improved in ablative technologies to mitigate this problem, and reduce healing times by shrinking the treatment area. This technology would come to be known as fractional skin resurfacing. Nonetheless, to this day, ablative laser skin resurfacing remains one of the most aggressive and effective type of photo-energy treatment, and carries the greatest risks of skin damage and painful recovery time.

Non-Ablative Skin Laser Resurfacing

Non-ablative, or non-wounding, laser skin resurfacing utilizes technology that will penetrate the dermis or underlying skin, but will not actually burn off cells from the surface. This type of treatment aims to heat up collagen fibers in the deeper layers, this stimulating protein development to tighten skin over the long term. If the ablative laser aims to simply remove skin imperfections, this weaker laser technology attempts to get rid of wrinkles from beneath. Compared to ablative laser skin resurfacing, these treatments are less effective but are safer and require less recovery time. Non-ablative laser skin resurfacing is usually performed on thinner skin areas, where the energy has fewer obstacles on its way to the dermis. This helps deliver treatment more efficiently and avoid damage to the epidermis. Such non-ablative laser technology is classified into three main groups namely: mid-infrared lasers, visible lasers and intense pulsed light.

Fractional Skin Resurfacing

Fractional laser skin resurfacing seeks to combine ablative and non-ablative techniques. It targets both the outer layer and the underlying skin by breaking the laser beam into smaller bursts of energy. Fractional laser skin resurfacing is used to treat acne scarring, skin pigmentation and wrinkles. With fractional laser resurfacing, it’s important to realize that it is not the laser itself that makes the difference, but how it is applied to the skin. The fractional technique uses the idea that the entire treatment area should not be damaged, but only a fraction of it. During treatment, the laser beam is divided into smaller “packets” of energy that are selectively delivered to parts of the treatment area, leaving healthy tissues in between. This fractional laser resurfacing idea has been used with ablative and non-ablative devices because it causes less damage, thereby decreasing side effects. The Fraxel laser device was the pioneer in fractional laser skin resurfacing. Now, many companies have developed their own versions, with various additions like suctioning technologies to increase light penetration into the skin, thereby decreasing the amount of energy needed; and specialized filters to better target specific skin problems.

Fractional Laser Resurfacing Side Effects

Although fractional laser skin resurfacing is less effective compared to ablative skin resurfacing, it is becoming a more popular choice because of the lower instance of side effects. However, this does not mean that fractional skin resurfacing is the best skin treatment. Side effects from aggressive treatments with this technique can include swelling, inflammation, hypersensitivity, and even skin peeling and crusting, which could last for up to a week after the procedure. Also, patients with darker skin tones, or those with a history of melasma, are prone to post-inflammatory pigmentation issues. Therefore, it’s important not to fall into the advertising trap for such procedures, which often make the side effects seem less serious, and to understand that fractional laser skin resurfacing has its own set of problems and potential health hazards for patients.

The last two decades saw a rapid development of laser resurfacing technologies to treat fine lines, get rid of wrinkles, and address aging skin, with the Erbium laser resurfacing becoming a viable alternative to more traditional skin ablation methods. The biggest problems with laser skin care are that it often causes significant damage to the skin and requires downtime after the procedure to recover. How the Erbium Laser Works

The idea behind the Erbium laser is straightforward; damage skin in a careful manner to stimulate the body’s natural regenerative processes. This is the same approach as most other laser skin resurfacing treatments take, but big differences appear when you focus on the details. For example, unlike an Erbium laser, the CO2 laser removes dead skin cells through a thermo-mechanical reaction. Here, the CO2 machine overloads skin cells with energy, causing them to vaporize. However, in this reaction, significant amounts of heat dissipate into surrounding skin tissue, causing additional damage that gives little benefit to the patient.

The Erbium laser works in a similar manner, except that it is photomechanical in nature, meaning that when targeted cells are destroyed, significantly less heat dissipates into surrounding tissues. Less errant heat means less damage to the face, hands, neck, or any other areas being treated. Furthermore, the Erbium laser delivers energy in pulsating bursts, as opposed to earlier CO2 models, which treated skin with a continuous beam, which causes even more damage. This makes the Erbium laser a more conservative treatment option which can shorten down time.

What the Erbium Laser Treats

The Erbium laser is designed to treat a variety of skin issues, in the same way a chemical peel would. The laser resurfacing action of this device eliminates damaged cells on the surface of the skin, allowing new skin to form in its place. Therefore, problems like pigmentation issues, acne scars, wrinkles, and liver spots can all be gradually diminished with several treatments.

However, this device is less effective for natural skin tightening because it does not stimulate collagen growth, and focuses strictly on treating the epidermis, and not the deeper layers of the skin. Alternative treatments like fractional laser skin resurfacing, ultra sound therapy, and radiofrequency options are likely to work better than the Erbium laser for collagen stimulation purposes, because they penetrate deeper and heat up the dermis without damaging the upper-most layers of the skin.

It is also difficult to treat deep wrinkles with an Erbium laser because it does not remove tissue aggressively enough in such instances. Not that the laser wouldn’t necessarily work for severe facial lines, but more treatments would be required, and results would not present themselves as quickly. However, newer versions of this technology may soon provide more effective for such problems by increasing the power bursts and the depth of the penetration into the skin, although the long term effectiveness of such treatments remains to be seen.

Erbium Laser Side Effects

Treatment with an Erbium laser is considered to be less intense than a CO2 laser, but a number of side effects and the need for recovery time still persist. It is not uncommon for individuals to experience inflammation in the treatment area and highly sensitive skin for several days. In addition, anyone dealing with infection at the treatment site should avoid the procedure, or any other laser resurfacing treatment, because the micro damage creates a risk of further infection. If you are dealing with inflammation (which is often a sign of infection) the Erbium laser treatment is likely a bad idea in such cases as well.

CO2 Laser

CO2 Laser Resurfacing

Laser technology has been used for several decades in skin care, and the CO2 laser is a very common procedure option in physician offices. This technology has been applied in the treatment of a variety of issues; from wrinkles around eyes, to dark spots on face, and acne scars not treatable with Dermabrasion. But its early popularity is what also caused the CO2 laser to see many changes over the years. For example, earlier versions of these devices were often accompanied by significant discomfort and side effects in the form of redness, inflammation, and peeling skin. As consumer demand for minimally invasive treatments increased, the technology evolved with the addition of various cooling mechanisms to increase patient comfort, and strategies to limit skin damage through a more methodical laser beam application. The CO2 laser treatment of today has improved significantly from its predecessors, but risks of skin damage and painful side effects have not disappeared completely.

CO2 Laser Technology

The CO2 laser creates infrared waves, which are then amplified through a series of mirrors and lenses to deliver a powerful energy beam to the surface of the skin. Physicians use specially-designed versions of this device to selectively damage skin tissues, a process called ablation, and spur the body’s natural healing process.

In the more traditional applications, the CO2 laser would fully burn off the top layer of skin in the entire treatment area, providing for a type of laser peel. Damaged skin cells were completely vaporized, and fresh, glowing skin was present beneath. The intensity of this procedure, however, is too painful for some patients to endure, and the CO2 resurfacing laser treatment was altered over time to use a fractional approach.

The fractional version of the CO2 laser, or fractional Co2 laser, is very similar to the initial models, except for one key difference: it doesn’t burn off the entire upper layer of skin cells. Instead, this type of device destroys only tiny patches of skin within the treatment area, leaving much of the surface intact. This improvement on the CO2 laser allows for faster healing, but at the same time, may also require more treatments to show meaningful results.

CO2 Resurfacing Laser Treatment

The use of the CO2 laser is most common for conditions that can benefit from removal of damaged skin cells. Hyperpigmentation issues, liver spots, medium to light wrinkles, fine lines and crow’s feet can all be addressed with laser resurfacing. Because the treatment literally burns portions of the skin, the need for a topical pain cream is highly likely, and in severe cases general anesthesia may be required.

However, this technology is not thought to be as effective at long term skin tightening because it doesn’t focus on collagen stimulation. This protein is found in the dermis layer of the skin, but the CO2 laser resurfacing treatment isn’t meant to go that deep. Further, collagen is best stimulated by being heated and coagulated, as opposed to being completely vaporized. Therefore, other technologies like Thermage and Sublative Rejuvenation may need to be utilized instead of a CO2 laser for long term anti ageing effects.

Side Effects of CO2 Laser Resurfacing

Any CO2 laser treatment is likely to cause discomfort and require 3-4 days to recover. However, some individuals may be at a higher risk because of previous medical problems. For example, patients with infections in the treatment area should not have the procedure as the micro wounds could easily become infected, exacerbating the problem.

Individuals who have undergone a chemical peel in the last few months, or who has had Dermabrasion performed is also not a good candidates for CO2 laser treatment. The idea behind many such treatments is the same as with laser resurfacing; to break down the already damaged cells on the surface and allow the skin to heal itself. If a CO2 laser procedure is performed soon after, the damage may be too severe for the body to easily handle and may lead to permanent skin discoloration or scarring.

Should I Consider a Neck Lift Instead of a Facelift?

If the area around the eyes is the first place to show signs of ageing then the neck is definitely the next in line. Neck bands also known as platysmal bands appear when the platysma muscle becomes thick, leading to the appearance of vertical stripes and horizontal creases across the front neck. These changes can be unsightly and are common concerns for patients consulting with plastic surgeons. Excessive fat and loose skin around the neck can be corrected with a neck lift surgery. A combination of liposuction and removal of excess skin can help you get rid of the saggy neck skin. Neck lift is sometimes considered an alternative to a facelift. What Is a Mini Facelift and Am I a Candidate?
